A. Description of the subject of the contract:

Subject of the contract:

Construction of "Technological Park Champions in Plastics", i.e. a building for research and development center of product and service design of Geo Globe Polska Sp. z o.o. sp.k. company. and construction of a storage hall with internal installations (water, sewage, central heating,mechanical ventilation, air-conditioning, fire hydrant and electric installations) and development of the land: internal roads, parking lots, pedestrian paths, unloading area, demolition of existing buildings, construction of non-building installations (water, sewage, rainwater, electric), reconstruction of clashing existing installations of land development (water, electric, lighting, external lighting) on plots no. ) on plots No. 1269/109, 1253/110, 1050/96, 1258/110, precinct 0029 Mikołów and construction of two exits from plot No. 982/96 to plot No. 1050/96, precinct 0029 Mikołów at Dzieńdziela 30 Street in Mikołów.
Detailed technical specifications can be found in the design documentation of the hall, which is attached to this inquiry.
The scope of work will include the comprehensive execution of construction works related to the construction of the hall in accordance with the construction project, including installations . The request includes preparation of the site and construction backup facilities, earthworks, laying of foundations, erection of structures, construction of roofing and curtain walls, construction of floors, installation of window and door frames, installation of electrical, water and sewage, central heating and ventilation systems. All construction work should be carried out in accordance with technical and construction regulations, applicable Polish Standards, as well as in accordance with the principles of technical knowledge and safety regulations, and under the supervision of a person authorized to do so.

B. The bid should include:

1. the total price with a breakdown into phases of each work.
2. material and financial schedule of works
3. terms and form of payment (payment broken down in stages as stipulated in the contract).
4. bid validity period min. 45 days.
5 Bidder's insurance.
6. all details of the company - full address, telephone number, tax ID.
7 References - min. 3 confirmed comprehensive project implementations.
8 Implementation date.
9 Warranty Conditions.
10. comprehensive scope of work (preparatory works, earthworks and foundation works, electrical installations, plumbing installations, finishing works including the social part, central heating).

Only bids that contain all of the above information and meet the conditions in Section C will be considered.

C. Conditions to be fulfilled by the Bidder:

(1) Contractors who meet the conditions for participation in the proceedings, i.e.:
1) have civil liability insurance in respect of business activities related to the subject matter of the contract for a guarantee amount of not less than PLN 4million,
2) have funds in a bank account or creditworthiness of not less than PLN 7million.

Requirement description

(1) A contract may not be awarded to a Contractor who has a capital or personal relationship with the Contracting Authority. A capital or personal relationship means a mutual relationship between the Contracting Authority or persons authorized to incur liabilities on behalf of the Contracting Authority or persons performing activities on behalf of the Contracting Authority in connection with the contractor selection procedure and the contractor, consisting in particular of:

1) participation in a company as a partner in a civil partnership or partnership,
2) holding at least 10% shares,
3) serving as a member of the supervisory or management body, proxy, attorney,
4) being in a marital relationship, in a relationship of consanguinity or affinity in the direct line, kinship in the second degree or affinity in the collateral line, or in a relationship of adoption, custody or guardianship. (2) The Contracting Authority shall reject the bid of the Contractor who is related to it by capital or personal relationship.

D. The bid must be submitted by 03/03/2021 in the form:

The Procuring Entity shall not be responsible for and shall not bear any costs caused by expenses or losses incurred by the Bidder in connection with visits and inspections of the performance site, or any other aspects related to the preparation of the bid.

E. Bid Selection Criteria:

Bid selection will be based on a point system. The maximum number of points to be earned in total is 100 points. Points will be distributed according to the following breakdown:

1. price - max 80 points.
2 Warranty - max 20 points.

It is required to specify the net price and the amount of VAT.

Description of scoring - warranty criterion:

- 3 years and above - 20 points.
- Between one and three years - 10 points.
- Less than one year - 0 points.

The investment is scheduled for completion in 2024. The contracting authority will not consider partial bids.

The Procuring Entity reserves the right to conduct final negotiations with the selected Bidder.

The contracting authority reserves the right to discontinue the proceedings.

Attached to the request is a model contract, which will be completed and signed with the selected bidder after the bid with the highest number of points is selected.

Attached to the inquiry is a template for non-capital ties - each bidder participating in the proceedings must complete such a statement.
