A. Description of the subject of the contract:

It is planned to expand the HM-1 production hall with a module 7.5m wide and 40.2m long in axes. The total building area of 316, 9 m² and cubic capacity of 23133.7m³. Detailed technical specifications are included in the design documentation of the hall which is attached to this inquiry.

The planned expansion will consist of two elements, a technical and storage room and a gatehouse. The technical and storage room of an industrial nature will be implemented in light steel technology. A gatehouse with a traditional masonry construction is planned for the left side of the extension. The project envisages a gatehouse of two stories with an internal staircase leading above the ceiling of the first floor.

The scope of work will include the comprehensive execution of construction works related to the implementation of the extension of the hall for a technical room with a gatehouse in accordance with the construction project, including installations . The request includes the preparation of the site and construction facilities, earthworks, laying of foundations, erection of structures, construction of roof slopes and curtain walls, construction of floors, installation of window and door carpentry, installation works of electrical, water and sewage, central heating and ventilation systems. The scope of work should also include the dismantling of the existing container gatehouse located at the front of the HM1 hall.

All construction work shall be carried out in accordance with technical and construction regulations, applicable Polish Standards and the principles of technical knowledge and safety regulations, and under the supervision of a person authorized to do so.

B. The bid should include:

1. the total price of the technical room, as well as for individual scopes of work, such as:
- preparatory works
- earthworks and foundation work,
- electrical installations
- plumbing,
- finishing works including social area
- central heating
With a breakdown of materials and labor.

In addition, the total price for the Porter's Room should be separated, as the Contracting Authority allows partial bids to be submitted by division: Technical Room and Porter's Room.

2. terms and form of payment (payment broken down in stages as stipulated in the contract).
3. bid validity period min. 30 days.
4 Bidder's insurance.
5. all details of the company - full address, phone number, tax ID.
6 References - min. 3 confirmed comprehensive project implementations.
7 Implementation date.
8 Warranty Conditions.
9 Comprehensive scope of work (preparatory works, earthworks and foundation works, electrical installations, plumbing installations, finishing works including social area, central heating).

Only bids that contain all of the above information and meet the conditions in Section C will be considered.

C. Conditions to be fulfilled by the Bidder:

1. is authorized to act in legal transactions in the Republic of Poland.
2. has the necessary licenses to perform the work and activities that are the subject of the contract.
3. has the necessary knowledge and experience.
4. is in an economic and financial position to perform the contract.
5. no bankruptcy proceedings have been initiated against the Bidder or for which no bankruptcy has been declared.

D. The bid must be submitted by 20.09.2017 in the form:

The Procuring Entity shall not be responsible for and shall not bear any costs caused by expenses or losses incurred by the Bidder in connection with visits and inspections of the performance site, or any other aspects related to the preparation of the bid.

E. Bid Selection Criteria:

Bid selection will be based on a point system. The cost of construction of the Technical Room and the Porter's Room will be billed separately. The maximum number of points to be earned in total is 100 points. Points will be distributed according to the following division:

1. price - max 40 points.
2 Completion date - max 30 points.
3 Warranty - max 30 points.

Description of the way of awarding points for fulfilling a given criterion for evaluation of the offer:

Net price of the cheapest offer
------------- x 40 = number of points
Net price of the tested offer

It is required to specify the net price and the amount of VAT with a breakdown of the price into materials and labor.

Description of scoring - criterion of completion date:

- Completion of work within 6 months - 40 points.
- Completion of work between 6 and 7 months - 20 points.
- Completion of work more than 7 months - 0 points.

Description of scoring - warranty criterion:

- 5 years and above - 40 points.
- 3 years to 5 years - 20 points.
- Less than 3 years - 0 points.

The Procuring Entity reserves the right to conduct final negotiations with the selected Bidder.
The contracting authority reserves the right to discontinue the proceedings.

Attached to the request is a model contract, which will be completed and signed with the selected bidder after the bid with the highest number of points is selected.
