A. Description of the subject of the contract:

Subject of the contract:<\/strong>
The subject of the contract is the supply of a new or used thermostat<\/p>\n

Technical specifications:<\/p>\n

- circulating medium - water
- heating-cooling unit stabilizing the temperature of tools in the minimum temperature range of 20-90 deg C
- minimum heating power 20kW
- machine not older than 10 years
- power supply 3 x 380-400V 50Hz<\/p>\n

B. The bid should include:<\/strong><\/p>\n

1. price together with delivery to Mikolow, Day Street.
2 Terms and form of payment.
3 Delivery terms.
4. bid validity period 30 days.
5. all details of the company - full address, phone number, tax ID.<\/p>\n

C. Conditions to be fulfilled by the Bidder:<\/strong><\/p>\n

1. is authorized to act in legal transactions in the Republic of Poland.
2. has the necessary licenses to perform the work and activities that are the subject of the contract.
3. has the necessary knowledge and experience.
4. is in an economic and financial position to perform the contract.
5. no bankruptcy proceedings have been initiated against the Bidder or for which no bankruptcy has been declared.<\/p>\n

D. The bid must be submitted by May 13, 2016 at 2:00 pm in the form:<\/strong><\/p>\n

The Procuring Entity shall not be responsible for and shall not bear any costs caused by expenses or losses incurred by the Bidder in connection with visits and inspections of the performance site, or any other aspects related to the preparation of the bid.<\/p>\n

E. Bid Selection Criteria:<\/strong><\/p>\n

Only bids that meet 100 % technical parameters will be considered.
Bid selection will be based on a point system. The maximum number of points to be earned in total is 100 points. Points will be distributed according to the following breakdown:
1. price - max 80 points.
2. availability of the machine (delivery date) - 20 points.<\/p>\n

Description of the way of awarding points for fulfilling a given criterion for evaluation of the offer:<\/strong><\/p>\n

Description of scoring criterion price:
Net price of the cheapest offer
------------- x 80 = number of points
Net price of the tested offer
It is required to specify the net price and the amount of VAT.
Description of scoring of the availability criterion (machine acceptance date) - 20 points for availability up to 1 month, each additional month of waiting is 5 points less.<\/p>\n

The Procuring Entity reserves the right to conduct final negotiations with the selected Bidder.
