Description of the subject of the contract:<br \/><\/strong><br \/><strong>Subject of the contract:<\/strong><br \/>The subject of the contract is the supply of a new or used vacuum-pressure TERMOFORMER<br \/>Scope of delivery:<br \/>Vacuum-pressure TERMOFORMER - 1 item.<br \/>Technical specifications:<br \/>- nominal thermoforming area 3000x2000mm<br \/>- machine suitable for thermoforming in vacuum-pressure technology<br \/>- max board thickness 12mm<br \/>- Thermoforming height up to 650mm<br \/>- power supply 3 x 380-400V 50Hz<br \/>- machine equipped with a system of servomotors for pressing the frame and upper punch to enable vacuum-pressure forming<br \/>- machine equipped with vacum ( vacuum) systems for the upper punch and molding chamber<br \/>- machine not older than 12 years<\/p>\n<p>Condition for submitting a bid for a used machine: in the 7 years preceding the date of the bidder's sale, the fixed asset has not been co-financed by Community funds or national funds constituting public or de minimis aid. A statement from the seller specifying the seller of the fixed asset, the place and date of its purchase will also be required.<\/p>\n<p><strong>B. The bid should include:<\/strong><\/p>\n<p>1. price including delivery to Mikolov, 30 Dzie\u0144dziela St.<br \/>2 Terms and form of payment.<br \/>3 Delivery terms.<br \/>4. bid validity period 30 days.<br \/>5. all details of the company - full address, phone number, tax ID.<\/p>\n<p><strong>C. Conditions to be fulfilled by the Bidder:<\/strong><\/p>\n<p>1. is authorized to act in legal transactions in the Republic of Poland.<br \/>2. has the necessary licenses to perform the work and activities that are the subject of the contract.<br \/>3. has the necessary knowledge and experience.<br \/>4. is in an economic and financial position to perform the contract.<br \/>5. no bankruptcy proceedings have been initiated against the Bidder or for which no bankruptcy has been declared.<\/p>\n<p><strong>D. Bid Selection Criteria:<\/strong><\/p>\n<p>Only bids that meet 100 % technical parameters will be considered.<br \/>Bid selection will be based on a point system. The maximum number of points to be earned in total is 100 points. Points will be distributed according to the following breakdown:<br \/>1. price - max 80 points.<br \/>2. availability of the machine (delivery date) - 20 points.<\/p>\n<p><strong>Description of the way of awarding points for fulfilling a given criterion for evaluation of the offer:<\/strong><\/p>\n<p>Description of scoring criterion price:<br \/>Net price of the cheapest offer<br \/>------------- x 80 = number of points<br \/>Net price of the tested offer<br \/>It is required to specify the net price and the amount of VAT.<br \/>Description of scoring of the availability criterion (machine acceptance date) - 20 points for availability up to 1 month, each additional month of waiting is 5 points less.<\/p>\n<p>The Procuring Entity reserves the right to conduct final negotiations with the selected Bidder.<br \/>The contracting authority reserves the right to discontinue the proceedings in the event of failure to obtain financing in the form of a loan for technological innovation.<\/p>\n<p><strong>We would like to inform you that as a result of the proceedings for the supply of a new or used vacuum-pressure thermoforming machine, the offer of the supplier has been selected: DG - Industries e.i.
